This is the basic treatment in Infertility which involves stimulation of ovulation by medication. Ovulation induction uses hormonal therapy to stimulate egg development and to induce ovulation in women who do not ovulate on their own - typically women with irregular menstrual cycles. The goal is to produce a single, healthy egg. A typical cycle begins with a baseline ultrasound on day 2 of the menstrual cycle prior to starting oral medications. Oral medication is then usually prescribed to be taken on days 2-6 or 5-9 of a menstrual cycle. A follow up ultrasound will be performed on day 11 to check for follicular development. Once the lead follicle(s) measure at least 18-20 mm in diameter, instructions are given for either timed intercourse or to take an injection is given which causes ovulation, and this is followed by intrauterine insemination.
Intrauterine Insemination (IUI) is procedure in which carefully prepared (washed) semen is placed within the uterus using a small catheter through the cervix (neck of the womb). The technique is used to get a high concentration of sperm in the reproductive tract to increase the chance of sperm reaching and fertilizing the egg. This is a basic fertility treatment indicated in couples with, Borderline sperm count, Decreased sperm motility parameters, Ejaculatory dysfunction or Unexplained Infertility Patency of the fallopian tubes is a prerequisite for this treatment. This procedure is preceded by ovulation induction. On ultrasound monitoring, when the lead follicle reaches 18-20mm, trigger injection (HCG) is given and the IUI is scheduled 36 hours later. It is a simple painless procedure done in the clinic. The procedure takes just a few minutes and daily activities can be resumed immediately afterwards.
IVF or In Vitro Fertilization is one of the most successful assisted reproduction technologies (ARTs) during which mature eggs from a woman's ovaries are removed and fertilized with a male partner's sperm, outside the body, in a laboratory. Since fertilization takes place in a glass culture dish in the lab, the process is called "in vitro," which is Latin for "in glass." The fertilized egg is then implanted in to the woman's uterus, where the embryo then develops under natural conditions. Since not every fertilized egg or embryo develops into a pregnancy, doctors usually fertilize multiple eggs, which are then frozen for later use, if required. One IVF cycle nearly takes four weeks to complete. A pregnancy test is performed after about two weeks of the embryo being placed in the uterus. Prior to commencement of the IVF cycle, a basic health check is mandatory. This assessment generally included pelvic ultrasound, hormonal assay, uterine cavity assessment (SHG/Hysteroscopy), catheter trial, laparoscopy (if required) and semen culture and analysis.
This recent innovative technology in Assisted Conception aims at improving the success rates in severe male factor infertility and repeated unexplained IVF failures. Sperms that appear to be superficially normal at ICSI magnification (400 times) may have defects in the head or surface and prevent penetration of the egg by the sperm. These defects cause fertilisation failures leading to IVF failures. IMSI magnifies sperms by 8000 times and based on motile sperm organellar morphology examination (MSOME), it enables recognition of subtle imperfections in sperms such as the acrosome, mitochondria and nucleus. With this technology, morphologically normal sperms can be selected for IVF or the ICSI procedures. Rainbow Fertility Centre takes the lead in using this most innovative technique, becoming the first centre in Andhra Pradesh to introduce this technology.
Frozen Embryo Transfer
The surplus embryos that have been frozen in the IVF cycle can be successfully transferred in a Frozen Embryo Transfer without having to undergo stimulation of the eggs or egg retrieval. It is performed when the fresh embryo transfer was abandoned (due to poor endometrium or hyperstimulation) or unsuccessful transfer or for next pregnancy. It is also done in an oocyte donation or embryo donation programme. Frozen Embryo Transfer cycle is a relatively simple procedure. The frozen embryos are thawed and replaced in the uterus in a natural cycle or a hormonally supported cycle to improve the lining of the womb. Frozen Embryo transfer can be done even after several years of freezing as the embryos are suspended in age and time. The success rate of a FET is early same as that of a fresh transfer and primarily depends on a woman's age, quality and day of freezing of the embryos and the endometrial receptivity.
Stimulation for ovaries for IVF involves production of a number of oocytes. All the eggs that are retrieved are fertilized with the goal of making number of viable embryos to allow better selection at the time of embryo transfer. After 2-3 good embryos have been selected, there may still be surplus good grade embryos that have the potential to survive freezing and are viable when thawed. Cyropreservation is done of sperm samples, oocytes and embryo.
Male factor accounts for about 30-40% of infertility in couples. Hence, a thorough evaluation of the male partner is as important as the female. Male fertility problems related to impotence, sexual problems and other are addressed, which include Consultation services, Semen analysis, Semen Cryopreservation and Sperm chromatin structure assay (SCSA).
Ultrasound and Endoscopy are done when the doctor wants to have additional information about the ovaries, uterus, and the fallopian tubes. Ultrasound is an indirect imaging method which gives certain amount of information regarding the pelvic organs. Endoscopy gives a doctor more information than the ultrasound. Laparoscopy is generally recommended by a doctor to make an accurate diagnosis when you have pain and symptoms of pelvic infection. Hysteroscopy is used to diagnose or treat problems of the uterus.
This recent innovative technology in Assisted Conception aims at improving the success rates in severe male factor infertility and repeated unexplained IVF failures.
Assisted hatching facilitates in creating a hole in or thinning the zona pellucida, helping the embryo in the hatching process. The procedure is performed just before embryo transfer and has shown to significantly improve implantation rates. It is a safe procedure and is highly recommended for frozen embryos where the zona is hardened due to exposure to cryoprotectants and extremely low temperatures.
Surgical Sperm retrieval
This is a minor surgical procedure to obtain sperms directly from the testes or the epididymis. This is done under local anaesthesia and it is a day care procedure done on the same day of egg retrieval.
Oocyte donation involves fertilizing the oocytes of an anonymous oocyte donor with the husband's sperm by IVF and then transferring the resultant embryo in to the uterus of the female partner.
This is very similar to the oocyte donation treatment. It differs in the only way that the donor oocyte is fertilized by a donor sperm through IVF and the resultant embryo is transferred in to the uterus of the recipient couple.
